Merck and Moderna have announced promising results from a trial of their personalized cancer vaccine aimed at preventing the recurrence of melanoma, potentially paving the way for new treatment options across various tumor types, reports BritPanorama.

The vaccine successfully helped prevent the return and spread of the disease among over 1,000 melanoma patients, all of whom had undergone surgery to remove localized tumors and were at high risk for recurrence. Moderna anticipates that thousands of melanoma patients could benefit from this vaccine shortly after regulatory approval.

Unlike traditional chemotherapy, which indiscriminately eliminates both healthy and cancerous cells, or immunotherapy that generally amplifies the immune response, personalized cancer vaccines direct the immune system’s focus solely on unique mutations found within a patient’s tumor. This targeted approach may revolutionize how melanoma and other cancers are treated.

According to Dr. Ryan Sullivan, director of the Center for Melanoma at Mass General Brigham Cancer Institute, the trial’s positive outcome signifies a significant advancement for the oncology field, suggesting that similar approaches could redefine cancer treatment strategies as funding and interest in the area potentially increase.

More trials target other cancers

Beyond melanoma, Merck and Moderna are conducting late-stage development of personalized cancer vaccines for various other tumor types, including non-small cell lung cancer, where surgical removal was performed. Both companies have several large clinical trials underway related to this.

The companies are also exploring combination therapies in mid-stage trials involving bladder and kidney cancers and early-stage studies targeting pancreatic and stomach cancers. Modena’s President, Stephen Hoge, indicated that numerous trial results are anticipated in the coming year or two.

Additionally, Roche and BioNTech are investigating a similar treatment, known as autogene cevumeran, in mid-stage studies involving colon and pancreatic cancer patients who have undergone surgery. Results from the colon cancer trials are expected in 2027, while the pancreatic trial outcomes are slated for 2031.

Dr. Robert Vonderheide, director of Penn Medicine’s Abramson Cancer Center, expressed optimism about upcoming results across various studies, reflecting hope that many will yield positive outcomes similar to the melanoma results.

How it works

The treatment, referred to as intismeran autogene, employs a regimen combining Merck’s immunotherapy drug Keytruda with a customized mRNA vaccine. This protocol involves multiple doses of both the immunotherapy and the cancer-specific vaccine, designed to prime the immune system to identify and attack the target cancer cells.

Previously, Keytruda is administered every three to six weeks for up to a year following surgery to mitigate the risk of melanoma recurrence. Initial trial data indicated that the incorporation of the mRNA vaccine significantly reduced the risk of cancer return by half and lessened the chance of its spread by 59% when compared to treatment with Keytruda alone.

As the trial continues, researchers remain focused on validating whether this combined treatment can extend patient survival rates. Initial findings show no new side effects associated with the addition of the vaccine.

Dr. Elizabeth Buchbinder, a melanoma expert at Mass General Brigham, noted that the selection of melanoma for this combination trial is rooted in the tumor type’s high mutation rate, which affords the immune system a better chance to identify and target the melanoma cells. She expressed hope for broad applicability across other cancer types that also display multiple mutations, suggesting that this area of research could lead to significant developments in cancer treatment.

As research continues, the momentum in personalized cancer therapies is poised to accelerate, reflecting a transformative phase in oncological care.
